Our Skilled Nursing Facilities offer both short-term skilled and long-term options that are patient-centered, and allow patients to live their most independent life possible. Our communities feature state-of-the-art rehabilitation centers and spacious, comfortable living spaces that are designed to provide the highest quality of post-acute care in the most comfortable setting possible, all while keeping our patients close to home.
In addition to helping our patients feel as comfortable and at-home as possible, our facilities also provide a wide variety of services for both our rehabilitation and long-term care residents.
Skilled Nursing & Rehabilitation
Our rehabilitation care program caters to patients who have recently undergone: surgery, suffered a stroke, cardiac incident, congestive obstructive pulmonary disease exacerbation (COPD), congestive heart failure exacerbation or are just finishing a hospital stay, but still need assistance or treatment to help them get back to their lives. Our rehabilitation and therapy treatments are offered in a comfortable environment for the duration of your stay.
- Physical Therapy: We will work to improve functional mobility, strength and coordination to increase independence with mobility, transfers and ambulation.
- Occupational Therapy: We help patients improve functional activities such as eating, bathing, dressing, cooking and doing laundry, etc.
- Speech Therapy: Our programs are designed to improve functional communication and swallowing skills.

Our long-term care program features an around-the-clock nursing staff who provide quality and compassionate care to patients who are unable to be cared for at home. The care we provide to our long-term care residents includes assistance with basic life functions such as getting out of bed, getting dressed, bathing or eating. All of our care is patient-centered; meaning everything we do has the patient in mind. This patient-centered focus means that throughout the treatment or care process, our patients will always be made to feel as comfortable and independent as possible, while also maintaining their dignity throughout the care process.
